Personal Experience

This is also from the perspective of using a good, reputable, piercer. People can and do use less professional, or sterile, means but they're not really recommended.
Preparation
First off you have to expose whatever the area is. Yes, it generally is quite embarrassing when it's a genital pierce unless you are an exhibitionist. To be fair, they really will have seen it all before and are just regarding it as day-to-day work, not something personal. As for male genital piercing, whether it is up or it is down, they can deal with it, so don't worry about it. The odds are you'll go in thinking, "Look big and impressive!" to which your [now former] best friend will attempt to hide and look as small as possible in fear of the needle - it's normal, don't worry about it.
The area to be pierced will be sterilised and the piercer, discussing with you, will usually mark dots for the entry and exit points, usually in purple ink. You can agree them, move them, do whatever. Try not to poke them with your dirty fingers as the piercer will have to re-sterilise.
Once you've agreed the location, for most pierces, a clamp will be put on to hold the site still. This pinches but it's not too bad. The clamp has holes in the centre of each jaw for the needle to go through later.
The Pierce
You'll usually be asked to take a deep breath and then let it out. As you exhale the pierce will push the needle in and out - it is very quick. The needle remains in the wound and the piercer then removes the handle from it and also the clamps. The jewelry is put in to the back of the needle and the needle is slid out. As the needle slides out, it pulls the jewelry through so you don't even feel the jewelry go in. That is it. It is over in less time than you have taken to read this paragraph.
Finally, the balls are put on the end, or in to the ring, on the jewelry. For a lot of pierces a dressing is put over it to keep it clean and protected during its initial healing and then you can leave, taking aftercare kits etc. with you as you go.
